Before your first appointment:

• Please complete a new client information form (Microsoft Word format), save it as a
  .doc, .docx, or .pdf file and email it to jackie@jackiemyland.com.
  (If you are using Mac or iCloud Pages, please be sure to save as a .doc file or print/save a .pdf file. Do not send the .pages file.)

  Or, print a new client infomation form (.pdf format), fill it in by hand, and mail or fax it to our business office:

• Please send us a copy of your health insurance card:
  If you have a scanner or digital camera, please email an image of your health insurance card to jackie@jackiemyland.com.
  Or, mail or fax a copy as above.

Be sure to bring your health insurance card to your first appointment.
